Just in time, now that we all may become a patient
Patient Activation through Community Conversations (PACC) is a program where people share their experiences so others can take away important information that would be of benefit to someone as a caregiver, someone recently receiving a new diagnosis or entering the healthcare system. PACC encourages finding solutions through what has worked for others.
This PACC will include topics from The TakeCHARGE Campaign with a focus on Step # 3 How do you prepare for the doctor visit? What has worked for you? Step # 2 will be reviewed: Keep a record of your medications and medical records. For those who have attended the past PACC's has anything changed? Any new questions or experiences for the group?
Registration to this and all Pulse CPSEA Zoom events is required. Speaking about medical care can become a very sensitive topic so we ask all participants to review the Disclaimer Notice before registering.

About the PACC:
- Choose from a list of topics you want to speak about or learn about from others experience - or choose your own
- Share only what has worked for you. Advice is strongly discouraged - a PACC is about solutions - Finding what works

A PACC is a facilitated discussion virtually (on-line) or in person to discuss patient experiences.
People learn best when sharing. For years Pulse Center for Patient Safety Education & Advocacy (CPSEA) has been speaking with groups of people about their experiences as patients or in using the healthcare system. In small groups, when people share their experiences, others learn.
This program is where people share their experiences so others can take away important information that would be of benefit to someone as a caregiver or someone recently entering the healthcare system. PACC encourages finding solutions through what has worked for others.
Actual conversation at a Pulse PACC Program with members of an HIV/AIDS support group
Facilitator: “Someone here chose this topic – ‘Do you have a healthcare proxy and how did you choose someone?' Who would like to discuss this?’”
PACC Participant: “I don’t have a healthcare proxy and don’t know how to choose someone if I don’t want them to know I am HIV positive. I would like to know what others have done.”
Do YOU have unanswered questions about your health care? Ever wondered “what you don’t know you don’t know”? PACC groups are the place to find out. They’re where people like you share what has worked for them, and what hasn’t.
Typical conversations may cover:
- Do you keep a list of your medications? If so where do you keep that list?
- What lists do you bring with you to a healthy check-up?
- How do you choose a doctor?
- Have you ever left your doctor’s office without seeing the doctor because you waited too long? Did you go back?
- . . . . and a wide range of other topics.

Minimum participants should be 10 people. Maximum 15.
PACC's can be virtual and offered on-line using a computer or phone with a camera and microphone.
Virtual PACC's are perfect for anyone who can't leave home or choose not to.
A PACC can be helpful to people who have the same or similar diagnosis, are close in age or have nothing in common except their use of the healthcare system.
People who attend PACC programs are committed to learning patient activation and empowerment skills and sharing what has worked for them so others can learn too.
Host a PACC
You name the place, reserve the space and send invitations. We do everything else including handle registration.
Train to be a PACC Facilitator.
